Nease - Pre-Excused Absence Form

A hard copy of this form needs to be turned into the front office at least one week prior to the absence – emails are not accepted.
District requires this form for college visits to be considered for pre-approval for a possible excuse absence. Please note - once a student reaches 15 absences in a school year, only a note from a physician can excuse absences.
This form can also be used to request pre-approval of non-Nease athletic competitions.
